If you're wondering whether your child might be autistic, you're probably discovering two things at once: the process is confusing, and the waitlists are long. Here's the honest map.
How diagnosis actually works
In Australia there's no single blood test or scan. Diagnosis is a clinical judgement made against the DSM-5 criteria, ideally by a multidisciplinary team — typically a psychologist alongside a paediatrician or psychiatrist, often with a speech pathologist's input. A comprehensive assessment looks at social communication, play, sensory patterns, and development history, using standardised tools plus observation and parent interview.
The waitlist reality
Public (free) assessment pathways through children's hospitals and community health can run 12 months or longer in Sydney. Private assessments are faster — commonly a few weeks to a few months — but cost anywhere from roughly $1,500 to $3,500 depending on who's involved. Some families start on a public list and book privately in parallel; there's no rule against being on both.
Here's the part many families don't know
You don't need a diagnosis to start therapy. NDIS early childhood pathways for under-9s can provide support based on developmental concern, and therapy addressing communication, regulation or daily-living skills works the same before and after a formal label. Waiting for the assessment is not a reason to wait on support — the early years are precisely when therapy compounds fastest.
What to do while you wait
Keep a short log of specific examples (what happened, where, how often) — assessors treat concrete observations as gold. Ask your child's educators to write down what they see. Book a hearing check to rule out a common confounder. After the assessment
A diagnosis (or a documented developmental delay) opens NDIS access, and the assessment report doubles as evidence for your access request. From there, supports like psychology, speech therapy and OT are funded under Capacity Building.
